Consent to activities

I understand and accept that scouting is a skilled and physically demanding activity, and I accept that there is a risk of injury when participating in activities. I hereby give my permission for my child to participate in all activities organised and run by Guides and Scouts of Europe- Ireland. I authorise the Scouters or other parents to transport my child by car.

I have read, understood and subscribe to the “Presentation and Educational Project of the Union Internationale des Guides et Scouts d’Europe Fédération de Scoutisme Européen (UIGSE-FSE)”, the Child Safeguarding Policy and Best Practice for Scouters, Health and Safety Policy of Guides and Scouts of Europe –Ireland. Code of Behaviour

I am aware and I accept that a breach of the Code of Behaviour, Rules and Policies of Guides and Scouts of Europe- Ireland by me or my child may result in him/her being asked to leave the activity or being sent home at my expense. I authorise, confirm and agree that the Scouters designated by Guides and Scouts of Europe- Ireland shall have authority over our child and the right to give lawful instructions to our child to the same extent as we ourselves would be able to do. I am aware and I accept that a continued breach by myself or by my child of the Code of Behaviour, Rules and Policies of Guides and Scouts of Europe- Ireland may result in this membership being cancelled.

Medical details & Emergency

I understand that if my child requires medical attention, all reasonable efforts will be made to contact me, or the Emergency Contact Person, at the numbers provided on this form. If my child is taken ill or injured during this scouting year, I hereby consent to emergency medical, surgical or dental treatment that may be necessary in a situation where I cannot be contacted for the purposes of giving consent at the time of treatment. I hereby authorise the Scouter designated by Guides and Scouts of Europe- Ireland to communicate our consent to any treating medical or dental practitioner. I confirm that the medical details provided about my child are correct, and undertake to provide updated information if necessary, during the scouting year. I consent to Guides and Scouts of Europe- Ireland holding my child’s medical information so that it may used, only when necessary, without prior permission, or unless required by law to protect my child.

Loss or Damage to personal property

I accept that Guides and Scouts of Europe- Ireland is not responsible for any loss or damage to personal property/belongings caused, during or connected with the activities organised by Guides and Scouts of Europe- Ireland and I waive forever all and any claims against Guides and Scouts of Europe- Ireland and its members in this respect, for myself and on behalf my child.

Personal data and information on application

I understand that the personal data on this form will be used by Guides and Scouts of Europe- Ireland for the purpose of registering (or re-registering), maintaining the applicant’s membership, and for communications concerning activities of Guides and Scouts of Europe- Ireland. I understand that the Personal Data will be retained by Guides and Scouts of Europe- Ireland for as long as the applicant remains a member. I understand that the applicant can resign their membership by writing to Guides and Scouts of Europe- Ireland and that all their personal data will then be erased, except for a minimum membership record for archival purposes. I confirm that the alternative emergency contact person has been informed that their data is given to Guides and Scouts of Europe- Ireland.
